General:
When we start a patient on the program (ORT – opioid replacement therapy), we will usually only increase one medication at a time. Medication combinations when increasing doses can be dangerous. So we will usually get the opioid dose stable, before adding in any other medication such as for sedation or anxiety.
Medications such as benzos (diazepam or Valium) and narcotics (buprenorphine, Subutex, Suboxone, methadone), multiply their effects when their added together. They do not add their effects when taken together. This means that we are very careful in using multiple medications, if we are increasing doses.
Many clinics have the clinic policy of “ONE PROBLEM --- ONE CONSULTATION”.
